Supporting You with DPP and DSMES Services

Our pharmacy is proud to offer two important programs to help you take control of your health: the Diabetes Prevention Program (DPP) and Diabetes Self-Management Education and Support (DSMES) services. The DPP is designed for individuals at risk of developing Type 2 diabetes and focuses on building healthy habits through nutrition, physical activity, and lifestyle changes to prevent or delay the onset of diabetes. For those already diagnosed, our DSMES program provides personalized education and ongoing support to help you manage your diabetes effectively, covering topics like blood sugar monitoring, medication use, nutrition, and problem-solving skills. Both programs are led by trained professionals who work closely with you to set realistic goals and celebrate your progress. Open enrollment is the perfect time to review your health coverage and make sure you’re getting the most from your benefits.
